Position : Licensed Master Social Worker (LMSW)Location : New York, NY 10065Shift : 9:00 AM - 5:00 PM (includes a 60-minute break)Department : Care Coordination and Social WorkJob Summary :
This is a 12-week assignment requiring a Monday to Friday schedule. The Licensed Master Social Worker will provide comprehensive social work services in an inpatient hospital setting, managing a caseload of 25 to 30 patients.Responsibilities :Conduct in-depth psychosocial assessments to evaluate patients' needs and challenges.Develop and implement individualized care plans in collaboration with healthcare teams.Provide support and resources for patients dealing with complex issues such as hospice care, hemodialysis, substance abuse, guardianship, and ethical cases.Maintain accurate documentation in accordance with hospital policies and procedures.Utilize EPIC and Allscripts systems for patient management and record-keeping.Advocate for patients and their families, ensuring access to necessary services and support.Qualifications :Master's Degree in Social Work (MSW) (required).Licensed Master Social Worker (LMSW) by the State of New York (required).Inpatient hospital experience (required).Experience with EPIC and Allscripts software (preferred).Strong psychosocial assessment skills and the ability to manage complex caseloads (required).
