The Wanda Alston Foundation is currently seeking TWO Case Managers to join their team for their Transitional and Extended Transitional Housing programs.Case Managers provides youth-centered, trauma-informed case management services for eight to twelve LGBTQIA2S+ individuals ages 18 to 24 who live in The Wanda Alston Foundation’s transitional and extended transitional living programs. The Case Manager addresses client needs such as connecting youth with medical and mental health services, accessing public benefits and community resources, developing budgeting skills, and connecting youth with permanent and stable housing. Managers work closely with Housing Monitors who staff the facility on a 24/7 basis to implement programming and assist in clients meeting their treatment goal plans.All Case Managers rely on clinical skills to develop and track individual service and treatment plans for each client.In addition to the above responsibilities, Extended Transitional Housing (ETH) Case managers work with queer youth with severe mental health issues, trauma and need for life skills support. ETH clients may struggle with Bipolar, Depression, Schizophrenia and/or also have a dual diagnosis of substance abuse.This role reports directly to the Operations Manager.Tasks include, but are not limited to:Develop and implement service and treatment plans for each client while prioritizing the goal of obtaining and achieving independent livingAssist clients in need of obtaining benefits, entitlements, and other legal servicesProvide crisis intervention services if needed and referrals for psychiatric assessmentCoordination of care with service providers including medical, mental health, substance abuse, and other supportive servicesProvide on-call support on a rotating basis with other on-call leadership team membersRespond to emergencies, call-outs, etc.Weekly in-person case management services for each residentDevelop and maintain of community partnerships to provide superior servicesFacilitate group life skills trainings with clientsOngoing assessment and adjustment of services provided in collaboration with other staff and community partnersAccurate and timely documentation and reporting of services through HMIS (Homeless Management Information System)Maintain a high level of confidentiality and integrity when handling client informationDesired Skills and Qualifications:Required: Bachelor’s degree in Psychology, Social Work, Psychology, Sociology, Counseling, or related service/science or healthcare plus a minimum of two years of relevant human service, social services, or case management experienceA current and active DC License required for candidates with a Master’s Degrees in Counseling, Social Work or Marriage & Family therapyExperience with Homeless Management Information System (HMIS) or comparable system and strong knowledge of case management documentation and record keepingExperience working in transitional living programs or similar housing programKnowledge of substance abuse/recovery, family systems, medical and mental health treatmentExperience working closely with LGBTQIA2S+ young adults and sensitivity to the needs and challenges of LGBTQIA2S+ young adults experiencing homelessness, at risk for homelessness, or those who have experience traumaKnowledge of community (Washington, DC metro area) providers and resources including LGBTQIA2S+ health care, mental health care, housing, employment, social and needs based services, transportation, and educational resourcesExcellent interpersonal and written communication skillsExcellent organizational and time management skillsSelf-starter who can multitask and efficiently prioritize tasksMust be able to exercise patience and understanding even in escalated or crisis situations
