Position SummaryAt Stony Brook Medicine, our

Social Workers

are valuable members of our team, whoprovide empathic care to our patients, their families and visitors, with the highest degree of integrity and respect. Social Workers apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings. Social Workers plan, implement, coordinate and document treatment for individuals diagnosed with: mental illness and serious emotional disturbance; mental retardation and developmental disabilities; work-related illnesses or injuries; chemical addiction; or perpetrators or victims of neglect, abuse, or crime who need therapeutic intervention, habilitation, training, supervision and re-socialization, or confinement.Qualified candidates will demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care and effectively respond to changing patient needs by making decisions based on ethical principles and adhering to our high standard of excellence.Duties of a Social Worker II may include the following but are not limited to:Psychosocial assessment of selected cases.Effectuate discharge planning.Attend rounds and coordinate with medical team.Work with community agencies in regards to patient post discharge needs.Work with SB Outpatient division for improved continuum of care.Compliance with documentation of Social Work intervention.Coordination of discharge transportation, insurance verification, advocacy with insurance companies for discharge planning and insurance reviews as needed.Assist patient financial services, benefits or with Medicaid applications as needed.Coordinate and give review to insurance companies as needed; contact insurance companies for authorizations.Assist patients and families with advanced directives.Supervision of professional social work students.Active involved in performance improvement activities including program development, data collection and analysis on palliative care cases.Coordinate with Psychiatry and intervention with psychiatric patients on medicine units.Cross coverage on units.Assist patients and families with advanced directives and MOLSTActive in performance improvement activities including program development, data collection, and analysis on palliative care cases.All other duties as assigned.

Qualifications:Required:

MSW from an accredited Social Work Education School. New York State LMSW or LCSW. Three years post Master's. Experience in medical health care setting. Demonstrates excellent written and oral communication skills. Demonstrates strong Psychosocial Assessment skill set. Demonstrates understanding of discharge planning and long length of stay barriers and systems. Working knowledge of Long Island and Metropolitan NY Community resources. Demonstrates patient first, patient advocacy and service excellence. Computer literate and understanding of data analysis.Demonstrates the following:

Understanding of community systems; demonstrates ability for project management and action planning; demonstrates team work and team leader focus; understanding of data collection and statistics. Ability to cover multiple patient population.Preferred:

Experience in medical specialty areas. Experience of three or more years in Hospital, Hospice, SNF, or other inpatient medical setting.Special Notes
