Formed in 1993 by the YMCA of Greater Cleveland, Y-Haven has helped thousands recover from addiction, secure permanent housing, and re-build their lives. Y-Haven offers healing and hope.POSITION SUMMARY:Provide individual counseling, group counseling and case management services to individuals with Substance Use Disorders (SUD) so they may achieve sustained remission from SUD, restore and increase healthy social connections, obtain stable income and permanent housing, and reduce their risk of homelessness. As a Level II Case Counselor, also conduct diagnostic assessments, review and sign off on Level I Case Counselors notes, and provide them with mentoring and support.ESSENTIAL FUNCTIONS:

Provide individual counseling, outpatient group counseling, intensive outpatient group counseling, and case management to homeless individuals with substance use disorders.Provide individual counseling and case management to a case load of program participants.Facilitate Outpatient Group Counseling and Intensive Outpatient Group Counseling.Record all service activities by entering progress notes in the Y-Haven Electronic Medical Record.Establish and maintain regular collaborative relationships with social service organizations and programs that residents may utilize for employment, housing, continuing recovery and other needs.Participate in daily team meetings, quality assurance meetings, in-service trainings and coordination meetings.Coordinate with Clinical Director on all critical clinical issues.Conduct Diagnostic Assessments.Review and sign off on Level I Case Counselor Notes.Provide mentoring to Level I Case Counselors.Other duties as assigned.QUALIFICATIONS:Education:• Bachelor’s Degree RequiredExperience:• Must have at least 2 years’ experience in SUD Counseling.Certification(s) & Requirements:• Minimum License: LCDCIII (Licensed Chemical Dependency Counselor III), LSW (Licensed Social Worker), LPC (Licensed Professional Counselor).• CPR and First Aid certification and Safety training required within 30 days of hire.• Must be free from sanctions or exclusions from the Office of the Inspector General (OIG) and the State of Ohio Department of Medicaid.Technical Skills and Knowledge:• Bilingual (Spanish/English) preferred, but not required.• Proven ability to meet minimum productivity requirements.• Must have successful experience facilitating SUD group counseling.• Must have extensive knowledge of Substance Use Disorders and other issues affecting the homeless population.• Must have basic computer skills, using Microsoft Office applications.• Should have working knowledge of OHMAS, CARF and VA program compliance standards.• Demonstrate commitment to the mission, cause of the YMCA, and uphold its values and ethics.• Ability to work effectively with people of different backgrounds, abilities, opinions and perceptions.• Willingness and ability to be a mandated reporter for any signs or suspicion of child abuse and neglect.PHYSICAL DEMANDS:• Talking: Expressing or exchanging ideas by means of the spoken word.• Hearing: Perceiving the nature of sounds at normal speaking levels with or without correction.• Sedentary work: Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.WORK ENVIRONMENT:• The worker is not substantially exposed to adverse environmental conditions (such as in typical office or administrative work).DISCLAIMER:The above statements describe the general nature and level of work only.
