Clinical Case Manager

Job Summary: The Clinical Case Manager at Paving the Way MSI is a full-time hybrid position, working under the supervision of a program manager or lead clinical case manager. This role involves providing direct trauma-informed case management support, advocacy, and counseling to primary and secondary victims of crime. The Clinical Case Manager ensures timely access to community resources, legal supports, and behavioral health services, contributing to the holistic well-being of consumers.

Major Duties and Responsibilities:

Clinical Therapy (50%):

Provide therapy services to consumers using trauma-informed, evidence-based interventions.

Conduct psychosocial assessments and trauma screenings.

Manage personal caseload and address the clinical needs of the clinic efficiently.

Perform emergency evaluations of consumers in crisis and facilitate referrals to more intensive treatment or psychiatric hospitalization if needed.

Document all services as directed by Paving the Way MSI and the Office of Victim Services.

Ensure timely completion of treatment plans and Individual Treatment Plans (ITPs).

Provide consultation to the medical director and other clinic staff to facilitate a coordinated continuum of care.

Case Management (50%):

Assess immediate crisis needs of consumers and address them using trauma-informed crisis intervention strategies.

Identify concrete and safety needs of primary and secondary victims of crimes.

Provide linkage to essential supports such as housing, emergency shelter, domestic violence arrangements, childcare, Temporary Protective Orders (TPOs), behavioral health services, medical care, food programs, and travel assistance.

Collaborate with clients to develop and maintain relevant and meaningful case management and safety plans.

Follow standard policies and procedures and participate in agency quality improvement activities, including patient satisfaction and clinical outcomes.

Monitor social media outlets related to the Victim of Crime Program.

Adhere to grant deliverables and program outcomes.

Skills and Competency:

Strong clinical skills and competency in working with victims of crime, including assessments (risk and safety), planning, and delivery of care.

Effective coordination of services, including natural supports.

Education and Experience:

Required: Master’s Degree in Social Work or equivalent.

License: LGSW, LGPC.

Preferred Experience: Two years of counseling using trauma-informed interventions, one year of case management experience, and one year working with victims of crime.
