Overview

The M Health Fairview Palliative Care team has an immediate opening for a LICSW Social Worker based out of the M Health Fairview Masonic Children's Hospitall. This role will support pediatric palliative care patients at the Masonic Children's Hospital and attached clinics.This role will perform both in person clinic visits and in person hospital consultations, as well as virtual/telephonic care of palliative care patients and families. This rolel will work with an interdisciplinary team on a consult basis.

This is a 1.0 FTE (80 hours per two week pay period), benefits-eligible opening.

Responsibilities Job Description

The LICSW completes mental health assessments, crisis evaluation and counseling and psychosocial assessment of patients, for post hospital discharge needs. Assists patients with short-term counseling and planning following medical crisis. Ensures appropriate decision makers are informed of and involved in treatment planning.

Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards.

Actively participates in creating and implementing improvements.

Evaluates psychosocial information (including social, psychosocial, emotional, financial, and other related factors) to determine an appropriate action/discharge plan.

Assists physician with diagnoses related to mental health and chemical dependency.

Documents patient information in EHR and/or other electronic system in accordance with program standards, department policies/procedures.

Assists patient/family in utilizing appropriate community resources.

Performs supportive counseling to assist patient/families in adjusting to disability and illness and for realistic planning for post hospital care.

Completes coding of assessments and evaluations for billing

Palliative Care Social Workers:

Provide coping strategies for people facing terminal illnesses so that the individuals may retain the best quality of life possible

Address a client’s physical, mental, social, and spiritual well-being in all disease stages, and accompany the client from diagnosis to cure / from diagnosis of their terminal illness onward.

Make sure patients have access to resources

Provide psychological and emotional support for families during this difficult time

Provide grief counseling and family counseling, and assist families by coordinating care details and research helpful resources and benefits

Clinic Specific impact:

Conduct and document diagnostic assessments in the outpatient setting and develop treatment plans

Provide psychotherapy focused on coping with serious illness in person or by telehealth

Provide education to colleagues regarding psychosocial needs of patients with serious illness including:

Consultation

Co-visits

Regular participation in daily rounds

Conduct educational in-services

Qualifications

Required

Education:

Master's degree in Social Work from an accredited school of social work

Credentials:

Licensed Independent Clinical Social Worker (LICSW) - MN Board of Social Work

Experience:

3 to 5 years hospital social work experience. This experience may be substituted with direct clinical experience.

Knowledge:

Knowledge of DSM IV categories and criteria.

Preferred

Experience:

Experience in palliative care is preferred but not required
