Summary:The Care Team Nurse is the clinical leader of the care team. Under the Supervision of the Pediatrician, Nurse Practitioner and Nurse Manager, the Care Team Nurse maintains smooth flow of operation, manages high risk and chronically ill patients, provides education to patients and families, provides direct care and laboratory services and is responsible for telephone and direct triage.
Job Responsibilities:Demonstrates basics of infection control including standard precautionsDemonstrate skills and knowledge directly related to age-appropriate care of pediatric patients as well as proficient knowledge of equipment and instrumentationAssess the physical, emotional, psychosocial and socio-economical needs of the client and family, and refer to appropriate organizations for further evaluation when appropriateProvide urgent triage care in the office as needed as well as urgent telephone triage and/or crisis managementUse EMR to completely and accurately documents nursing care including triage, education, medication, reconciliation, and care planning when appropriateAct as facilitator of communication between family, physician, front desk and other health professionalsAdminister vaccines and medications, and perform in-office procedures such as blood work and catheterization in an efficient and professional manner, delegate appropriate tasks to Medical Assistant and other staff when appropriateContact patients and families after Emergency Room ad Hospital discharge to prevent readmission and post-hospitalization complications. Coordinate with home care services if necessaryMaintain pediatric-centered continuing education and trainingAdhere to the general policies and procedures as outlined by the practiceAbility to meet the necessary physical demands of the position

Other Qualifications:Excellent written and verbal communication, excellent organizationAbility to work independently as well as function as part of the care teamAbility to handle sensitive medical information/HIPAA complianceWorking knowledge of EMR, Microsoft office and emailEducational Requirements:Graduation from an accredited nursing program. The nurse requires a current license to practice nursing in the Commonwealth of Massachusetts by the Department of Public Health Nursing Certification Board as either a Registered Nurse (RN) or Licensed Practical Nurse (LPN). The nurse must also maintain Basic Life Support (BLS) certification and Continuing Educational Units (CEU's) as required for licensure. S/He must also practice within the guidelines of the American Nursing Association Standards or Practice.
