Outpatient Clinics - EAP
Rate of Pay:
$73.22/hour + DOE
Summary Provides medical care to patients with Substance Use Disorders through a comprehensive patient centered, team- based approach to care. Responsibilities include diagnosing, management of treatment, symptom management and pertinent health care maintenance.
Essential Duties and Responsibilities
Provides outpatient Addiction Medicine care and offers support to patients with substance use disorders using evidenced-based practices. Collaborates directly with the addiction medicine team and all members of the patient care team as well as outpatient community programs. Develops individualized treatment plans in collaboration with the Addiction Medicine team to achieve mutual goals using tools such as motivational interviewing and harm reduction strategies. Performs diagnostic evaluations which include effective patient interviewing techniques, obtaining health histories and performing assessments. Prescribes medication, therapy and other medical care per scope and standard of care. Communicates effectively with patients and or/family so they understand their diagnosis, treatment plans and need for follow up care. Uses clinical judgment and provides ongoing diagnoses throughout the process of treatment. Documents care rendered in the electronic health record (EHR). Provides feedback to the referral source if applicable via the EHR. Educates and encourages patients to become active participants in their own healthcare-related behavior and provides patient education materials when appropriate. Documents within the electronic medical record system to facilitate ongoing quality of care in a timely manner. Distinguishes between normal and abnormal findings. Makes appropriate referrals to primary care, other specialists and community agencies as needed. Collaborates with and consults the supervising physician (s) when appropriate and required by policy and completes chart review as required by health system policies. Escalates care appropriately when cases exceed training or expertise. Assists with EHR in basket coverage and patient triage. Performs all grant required duties and follows grant requirements. Adheres to all relevant policies, procedures and practice guidelines. Supports appropriate billing for professional services through timely documentation and maintaining adequate knowledge of the practice's billing policies and procedures. Responds to telephone calls and EMR messages in a timely manner. Regularly attends and actively participates in clinic staff and department meetings as requested. Stays up-to-date on best practices in Addiction Medicine and applies to clinical practice. Participates in community education and other professional activities to promote practice growth and community wellness as requested. Maintains positive, caring and supportive relationships with patients, care team and clinic staff. Demonstrates System Values in performance and behavior. Education and Experience Nurse Practitioner
Bachelor’s degree (B.S.) from four-year college or university; Graduation from a National League of Nursing (NLN), Commission on Collegiate Nursing Education (CCNE) or equivalent accredited school of nursing. The nurse shall have completed a nursing education program that included clinical experiences with actual patients under the supervision of qualified faculty. Must be certified by the American Academy of Nurse Practitioners (AANP) or the American Nurses credentialing Center (ANCC).
Physician Assistant
Completion of a Physician Assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant.
Licenses/Certifications
Nurse Practitioner
Current Advance Practice RN (APRN) licensure to practice in California (and/or NV, depending upon the position being sought) Current ANCC (American Nurses Credentialing Center) or AANP (American Academy of Nurse Practitioners) certification required. Current PNCB (Pediatric Nursing Certification Board) or current ANCC is required if requesting to work in pediatrics. Current BLS (Basic Life Support) and must maintain certification at all times. Must have a Collaborative Service Agreement (CA) or evidence of Supervising Physician Agreement (NV State Medical Board), as applicable, with a designated supervising physician member of the Hospital’s medical staff (upon hiring, manager will initiate). 5) Current, unrestricted DEA certificate in CA and/or NV, as applicable (Schedules II-V). Nevada Pharmacy Board certificate, if applying to work in NV. Ability to participate in a federally funded program (Medicare and Medicaid). Physician Assistant
Current CA and/or NV State license in good standing Current NCCPA (National Commission on Certification of Physician Assistants) certified Current BLS (Basic Life Support) and must maintain certification at all times Must have a Collaborative Agreement (CA) or evidence of Supervising Physician Agreement (NV State Medical Board), as applicable, with a designated supervising physician member of the Hospital’s medical staff (upon hiring, manager will initiate). Current, unrestricted DEA certificate in CA and/or NV, as applicable (Schedules II-V). Nevada Pharmacy Board certificate, if applying to work in NV Ability to participate in a federally funded program (Medicare and Medicaid); Physician Assistants must complete an educational course in controlled substances that meets the standards of practice by TFHD and the State of California.
