The Community Care Manager is responsible for increased revenue, establishing relationships with referral sources, and coordinates delivery of services for clients. Responsible for opening new cases and ensuring that care plan is appropriate for level of care needed. Provides support with planning, directing and organizing the delivery of services in a manner that ensures quality standards consistently meet or exceed the established philosophy, expectations and goals of the company.ESSENTIAL DUTIES:Meets or exceeds financial goalsSome travel requiredTakes initiative to create and implement programs to increase revenue and client populationResponsible for the care coordination of clientsEstablishes and maintains relationships with referral sourcesMonitor care plans to ensure client needs continue to be metCoordinate client weekly billing and transmittalPerforms supervisory visitsResponsible for scheduling staffing needs for all convenience servicesConducts caregiver orientation/trainingCoordination of client and caregiver schedules, approval of time slips and entering corrections/changes in scheduling softwareCaregiver coaching and counseling as requiredAddresses client and/or caregiver concerns and complaints in a timely manner. Performs investigation as needed.Ensures timely submission of all required caregiver paperworkReadily available via cell phone for on-callSupervises staff in Director’s absenceMaintains a high degree of confidentiality at all times due to access to sensitive informationMaintains regular, predictable, consistent attendance and is flexible to meet the needs of the departmentFollows all Medicare, Medicaid, and HIPAA regulations and requirementsAbides by all regulations, policies, procedures and standardsPerforms other duties as assignedPERFORMANCE RESPONSIBILITIES:Maintains positive internal and external customer service relationshipsMaintains open lines of communicationPlans and organizes work effectively and ensures its completionMeets all productivity requirementsDemonstrates team behavior and promotes a team-oriented environmentActively participates in continuous quality improvementRepresents the organization professionally at all timesPOSITION REQUIREMENTS & COMPETENCIES:Associate’s degree in related field5 years health care experience preferredCertified Nurse’s Aide in good standing with the state is preferredStrong communication skills and interpersonal skillsAbility to work independentlyValid driver’s license and proof of insurance is requiredTo apply via text, text 4923 to 334-518-4376#ACADCOR #CBACADCOR #DJADCOR
