Registered Nurse to join a growing and dynamic team!Job Description:Perform a thorough nursing assessment of the patient, including evaluating the caregivers' needs. Develop a care plan in collaboration with the interdisciplinary team, incorporating skilled nursing judgment, medical necessity, measurable objectives for both patient and caregivers and realistic goalsMake regular follow-up visits as deemed appropriate in the plan of careVisit patients daily (within a hospital setting or at their home)Empower and support caregivers and patients through education on the disease process and prescribed care regimensUtilize principles of palliative and hospice philosophy to provide holistic careProvide direct supervision of Certified Nursing AssistantPosition Requirements:Excellent observation, critical thinking, assessment, nursing judgment, and communication skillsPossess an interest and desire to give hospice care with a special sensitivity to the needs of the seriously ill populationSound working knowledge of hospice Conditions of Participation (COPs) and clinical standards of practiceAbility to maintain professional boundariesKahu Malama Nurses
