Job Summary:

As a Registered Nurse (RN) in Long Term Care, you will be responsible for providing comprehensive, patient-centered care to residents in a long-term care facility. You will work closely with other healthcare professionals to ensure the highest quality of care and improve the overall well-being of residents. Your role will involve administering medications, managing care plans, and offering emotional support to both residents and their families.

Key Responsibilities:

Patient Care:

Assess and monitor patients’ health conditions and vital signs.

Administer medications and treatments as prescribed.

Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.

Perform wound care, catheter management, and other specialized nursing tasks.

Care Coordination:

Coordinate with interdisciplinary teams to ensure comprehensive care.

Communicate effectively with residents, families, and healthcare providers about care plans, progress, and changes in health status.

Document all aspects of patient care, including assessments, interventions, and outcomes, in electronic health records.

Emergency Response:

Respond to medical emergencies promptly and efficiently.

Provide first aid and emergency care as needed.

Resident Support:

Offer emotional support and education to residents and their families regarding health conditions, treatment options, and care routines.

Promote a comfortable and safe environment for residents.

Compliance and Standards:

Adhere to facility policies, state regulations, and federal guidelines.

Ensure all care provided is compliant with safety and quality standards.

Education and Training:

Mentor and supervise nursing assistants and other junior staff members.

Participate in continuing education and professional development activities.

Qualifications:

Education and Licensure:

Associate's Degree in Nursing (ADN) or Bachelor’s Degree in Nursing (BSN) from an accredited institution.

Current and valid Registered Nurse (RN) license in [State].

Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification may be preferred or required depending on facility.

Experience:

Previous experience in long-term care or a related field preferred.

Demonstrated ability to manage complex care situations and make critical decisions.

Skills and Competencies:

Strong clinical assessment and problem-solving skills.

Excellent communication and interpersonal skills.

Ability to work collaboratively in a team environment.

Proficient in using electronic health records and other healthcare technologies.

Physical Requirements:

Ability to lift and move patients as needed.

Capability to stand, walk, and bend for extended periods.

Working Conditions:
