Job

Description/Requirements:

Evaluation/Treatment

of

Patients

in

their

home.

Driving

is

involved

in

this

position.

Assist

in

evaluating

the

level

of

function

and

in

developing

the

plan

of

care.

Evaluate

the

home

environment

and

makes

appropriate

recommendations

This is an independent contractor position and the

pay is $95-120 per

visit.

Potential

to

make

$150,000-

180,000
