We are currently seeking a skilled and motivated Physical Therapist to join our dynamic team. Job Summary: As a Physical Therapist, you will play a crucial role in assessing, planning, and implementing individualized rehabilitation programs for our long-term care residents. Your expertise will help improve mobility, strength, and overall function, ensuring our residents achieve the highest possible quality of life. Key Responsibilities: Conduct thorough assessments of patients physical conditions and needs. Develop and implement personalized treatment plans to address patients goals and needs. Provide direct therapeutic interventions, including manual therapy, exercise therapy, and modalities. Monitor and evaluate patient progress, adjusting treatment plans as necessary. Collaborate with a multidisciplinary team to provide holistic care. Educate patients and their families on treatment plans, exercises, and home care techniques. Document patient progress, treatment plans, and outcomes accurately and timely. Participate in care planning meetings and contribute to the development of care strategies. Ensure compliance with facility policies, procedures, and regulatory requirements. Qualifications: Degree in Physical Therapy from an accredited institution. Current and valid Physical Therapy license in Minnesota. Previous experience in long-term care or a similar setting preferred. Strong knowledge of physical therapy techniques, modalities, and interventions. Excellent communication and interpersonal skills. Ability to work collaboratively within a multidisciplinary team.
