Job Summary:
Completes psychosocial assessments on inpatients who meet high risk criteria. Discharge planning, crisis intervention, short-term counseling, resource information and referral are included in the Social Worker’s duties. Assists patients and physicians with information, applications, etc., when prescription assistance is needed.Qualifications/Reporting:Required minimum education:
Masters degree (MSW) from an accredited School of Social Work. Licensed by the Texas State Board of Social Work Examiners as a Licensed Master Social Worker (LMSW). Must maintain continuing education as required by the State Board of Social Work Examiners for licensure renewal annually.Required minimum experience:
Three to five years post graduate experience; preferably in a medical setting including interface with case management, continuing quality improvement and counseling experience. Management experience and/or education to work with patients of all ages is preferred.Required skills (if applicable):
Knowledge related to psychosocial issues associated with the disease process and of human behavior. Leadership and management skills and effective oral and written communication skills.Required license/certification:
Licensed by the Texas State Board of Social Work Examiners as a Licensed Master Social Worker (LMSW). Must maintain continuing education as required by the State Board of Social Work Examiners for licensure renewal annually. Current driver’s license and vehicle liability insurance.Job reports to (title of the position):
