VNS Health Behavioral Health team members provide vital client-centered behavioral health care to New Yorkers most in need, across all stages of life and mental well-being. We deliver care wherever our clients are, including outpatient clinics, clients homes, and the community. Our short and long-term service models include acute, transitional, and intensive care management programs that impact the most vulnerable populations, from children, to adolescents, to aging adults. What You Will DoPractices in the home and community to provide behavioral health treatment.
Conducts psychosocial assessments and living conditions of patients and social determinants of health and develops treatment recommendations.
Provides advocacy, psychoeducation, cognitive/behavioral interventions, monitoring of mental status, emotional support, brief psychotherapy, and/or family-based interventions.
Collaborates with interdisciplinary team members to achieve successful patient outcomes.
Licenses and Certifications:Current license to practice as a Registered Nurse in New York State, required
NYS non-Driver photo ID card may be required
Education:Bachelors or Associates degree in nursing required, masters degree in psychiatric nursing, preferred
Work Experience:Minimum of one year of recent inpatient, outpatient or home care psychiatric experience Valid driver's license or Strong clinical assessment and critical thinking skill
Excellent communication and interpersonal abilities
Ability to work independently and as part of a team
