This position is also eligible for up to a $2,500.00 sign on bonus. You must be employed in the exact position at time of payout.

As case managers, our therapists are able to establish strong relationships with patients and families and consult with physicians and other health care providers as necessary.
What you will need:
Licensed in the State of Illinois (or eligible for licensure).Completed 1 year of clinical experience or demonstration of strong spectrum of clinical affiliation experience.Graduated from an Occupational Therapy curriculum accredited by the American Occupational Therapy Association and current OT license in Illinois.Ability to work independently in non-acute setting, patient homesCommunicates professionally both verbally and through written reports, with strong documentation skills, along with good interpersonal skills. Demonstrates good time management and self-organization skills.What you will do:Demonstrates proficient use of all home based therapy equipment and assistive/adaptive devices (balls, bands, weights, dressing aids, adaptive devices, etc.).Providing treatment on standard beds in the home environment, Assesses, plans, implements and evaluates occupational therapy treatment programs established for the home care patient, as well as facilitates the progression of the patient in their home toward optimal levels of self-care.Assess, plan, implement and evaluate occupational therapy treatment programs established for the home care patient Facilitate the progression of the patient in their home toward optimal levels of self care.

Facilitates the patient and family's right to receive quality cost-effective care following Advocate's MVP of Stewardship .Care and Service Coordination : Formulates an integrated approach to care following Advocate's MVP of Partnership.Leadership/Collaboration/Professional Development : Demonstrates respect and understanding for peers and participates as an effective member of the patient care team to formulate an integrated approach to care following the Advocate MVP of Excellence.Patient Relationship : Develops and maintains a restorative relationship throughout the healthcare continuum, following Advocate's MVP of Compassion.Business Development and Growth : Demonstrates productivity and an understanding of financial and economic conditions in the health care environment.Patient Education : Demonstrates a commitment to meet the learning needs of patients and their families.COMPENSATION:
