Job Details:
As a Permanent Patient Care Coordinator, you will play an integral role in our manufacturing industry team. This is a temp-to-hire position that requires a minimum of 2 years of experience. The successful candidate will be responsible for coordinating patient care activities and ensuring that all patient needs are met in a timely and efficient manner. This role will involve working closely with other healthcare professionals, delivering high-quality patient care services, and maintaining accurate patient records. The candidate will also be responsible for ensuring compliance with all relevant healthcare regulations and standards.
Responsibilities:
- Coordinate all patient care activities, including scheduling appointments, managing patient records, and ensuring that all patient needs are met.
- Work closely with other healthcare professionals to deliver high-quality patient care services.
- Maintain accurate and up-to-date patient records.
- Ensure compliance with all relevant healthcare regulations and standards.
- Handle patient queries and concerns in a professional and empathetic manner.
- Collaborate with the healthcare team to develop and implement patient care plans.
- Facilitate communication between patients, family members, and healthcare professionals.
- Participate in patient care meetings and provide updates on patient progress.
- Identify opportunities for improving patient care services and make recommendations accordingly.
- Coordinate patient discharge processes and provide post-discharge support as necessary.
Qualifications:
- A minimum of 5 years of experience in a patient care coordination role or a similar position.
- Strong computer skills, including proficiency in using healthcare management systems and Microsoft Office Suite.
- Excellent communication and interpersonal skills, with the ability to interact effectively with patients, family members, and healthcare professionals.
- Strong organizational and multitasking skills, with the ability to manage multiple patient cases simultaneously.
- Knowledge of healthcare regulations and standards.
- Ability to work effectively in a team, as well as independently.
- Strong problem-solving skills, with the ability to make effective decisions under pressure.
- High level of empathy and understanding, with the ability to provide compassionate patient care.
- Ability to maintain patient confidentiality and handle sensitive information with discretion.
- Bachelor's degree in healthcare administration, nursing, or a related field is preferred.
