The DeKalb Regional Crisis Center provides crisis intervention and stabilization services for adults. The Center offers three levels of care starting with the Crisis Services Center unit that receives walk-ins, transfers from emergency rooms, and involuntary referrals for evaluation.If further observation is needed, individuals are monitored in the Temporary Observation Unit for 12-24 hours. Individuals needing a longer period of stabilization or substance withdrawal management (usually 5-7 days) are admitted to the Crisis Stabilization Unit where ongoing care includes crisis counseling, medication, and assistance with discharge planning.JOB SUMMARYUnder general supervision, the Registered Nurse provides professional nursing services in accordance with medical protocols. Assesses the patient's/consumer's condition, develops care plans, and provides treatment and education. Incumbent will be responsible for nursing assessments, administering injections, obtaining lab work, facilitating nursing groups, and assuring that consumers are authorized and eligible for nursing services prior to service delivery. Records pertinent observations and responses to treatment. Incumbent must be able to multi-task. Full-time staff will be scheduled for at least 4 weekend shifts during a month. These four shifts are not necessarily set at every other weekend, it could be 3 Saturdays and a Sunday, or alternating weekends, or two weekends in a row, etc., based on your scheduling needs and that of the unit.DUTIES AND RESPONSIBILITIESAssessmentsPerforms bio psychosocial assessment of clients and documents assessments within agency guidelines```{=html}``` - Observes clients, charts, and reports changes in client condition including adverse reactions to medications and/or treatment and follows up as needed. - Measures and records clients' vital signs including height, weight, temperature, blood pressure, pulse, and respirations - Works as part of a health care team to assess clients needs and plans, modifies, and implements interventions. - Performs duties from a recovery oriented, client centered, and trauma informed perspectiveCommunicationAlways exhibits clear and professional communication with clients and other staffProvides health related training/education for consumers and families as indicated by assessments or at treatment team requestCommunicates information clearly to physicians and to other providers as needed to ensure optimal client careEffectively communicates to families and other DCSB programs, contractors, and/or community partners (hospitals, clinics, MH or SA providers) as needed to promote client careComplex Case ManagementCoordinates medical and psychiatric care with contractors/community providers ensuring appropriate referral documentation and follow-up from consultationEnsures effective use of medication in clients plan through documentation of administration on the MAR, observing client use of medication in clients self-administration of medication (when indicated), administering medications (when indicated)Facilitates collection of laboratory specimens to include ensuring proper completion of the receiving facilities requisition formNursingProvides basic to advanced client care and treatment, such as wound dressing, administering medication, and venipunctureDemonstrates knowledge of normal and abnormal lab valuesAssess and documents response to interventions and modifies intervention as neededPerform chart checks each shift and document all medication errors according to agency policy and notify the Charge/Lead Nurse, APRN and Nurse ManagerComplete seclusion and restrain packets when applicableAdminister emergency medication and document accordingly in chartRespond appropriately to crisis situations that may involve physical intervention including the use of sec usion/restraint, intervene, stabilize and manage acute crisis situations as needed.Assist with admission/discharges.Review/Update BHL Board dailyMaintain narcotic count sheets are accurateAdminister PPD to new admits, read results and document in AvatarAdminister prescribed medications and document appropriately/accurately on MARAnswer clients calls and determine how to assist themMeasure and records clients vital signs, such as height, weight, temperature, blood pressure, pulse, respiration, and pain.Update daily logs on the unit (i.e., refrigerator temps, water temps, glucometer logs)Observe individuals, charting and reporting any changes in condition, such as adverse reaction to medication or treatment, and taking necessary action and report any concerns to the Charge/Lead RN, Nurse Manager and APRNProvides education to individuals and familyAssist in training co-staff in areas relating to service specialties as needed.Demonstrates competency for Blood Glucose Monitoring, Alcohol Breathalyzer, Urine Drug Screen, Pregnancy Test, PPD skin tests and in performing safe phlebotomy practices when collecting blood.Monitors and documents all ordered labs, screenings, and results such as PPD, Blood Glucose, UA, RPR, Blood Chem, and CBCAttend case staffing/treatment team meetings as directed by Nurse Manager.Accurately transcribe all verbal and written orders by the APRN/MDOther duties as assignedMINIMUM QUALIFICATIONSA current Georgia license to practice as a Registered Professional NurseMust have basic computer and typing skillsPREFERRED TRAINING and EXPERIENCEExperience in Mental Health preferred
