Purpose

Job Description

Under the general administration of the Chief of Ambulatory Care, collaborates with and is accountable to the Physician for health care assessment. Identifies, diagnoses and treats human responses for actual or potential health care problems for individuals and families. Provides such services as case-finding, health counseling, implementation and evaluation of primary health care as well as management of and referral follow-up of identified care gaps; Plan, organize, direct, coordinate and evaluate primary health needs and addressed urgent/emergent health needs for Ambulatory Care practices at both South Brooklyn Health & Ida G Israel.

Specific Duties And Responsibilities

The Nurse Practitioner is charged with and responsible for:

Perform History & Physical examinationsOrder routine diagnostic testing, e.g., laboratory, radiologic, etc.Ongoing medical documentation in the medical record, e.g., progress note, etc.Refer for appropriate consultation (s).Address identified patient care gaps and refer to additional imaging or specialty care as needed.Order medications and write prescriptions subject to Section 6902 (3) (b) of the New York State Education Law.Perform minor procedures and diagnostic testing, e.g., EKG, IV, phlebotomy, injections, NG tube, urinary catheterization, suturing, cultures, etc.Education and counseling of patients and families.Perform more advanced procedures and ACLS with documentation of appropriate training and documentation of competency.Medication refillsOther duties as required within the scope of practice

Minimum Qualifications

A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); andCurrent certificate as a Nurse Practitioner issued by the NYSED; andHolds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).

Department Preferences

Strongly prefer National Board Certification in specialty in which you are assigned
