Physical Therapist traveler needed primarily for home care environment with support from a large interdisciplinary team (IDT) within the PACE model of care.
40 hours per week
M-F, 8AM-4:30PM
PACE is a Program for the All Inclusive Care for the Elderly, utilizing dual benefits to provide preventative and home based care services to at-risk, frail 55+ elders in the community and prevent LTC placement.
PT will work closely with Rehab team and IDT to provide home care services.
No OASIS or billing restrictions to care as PACE is both the health care provider and the insurance provider.
Fantastic facility and very traveler friendly! This position will be to replace a traveler currently on assignment.
