Job Description

The ADON ensures that the health care needs of clients are met, that their staff is trained in home care policies and procedures, and remain in compliance with the most current comprehensive home care regulations. The ADON works alongside the Director of Nursing and LPN to meet the needs of the clients and their families and provide excellence of service to them.

Essential Job Functions

Oversee in partnership with the Director, HHA employees to ensure compliance with federal, state, local standards and regulations.

Maintain knowledge of current Home Care trends.

Investigate reports/incidents of suspected client neglect and/or abuse and report these findings to the Director of Nursing (D.O.N) and/or an appropriate county and/or state official.

Maintain confidentiality of clients and all personnel.

Record client data in electronic charting system (Eldermark).

Communicate/document client data to staff (as needed).

Follow the Comprehensive Home Care regulations.

Implement service agreements with all new clients. Review and revise service plans and care plans (as needed).

Monitor client charts and nursing notes.

Communicate any changes in client's needs/condition to physicians and other personnel (county caseworkers).

Renew medication as necessary.

In partnership with the Director and LPN, ensure that all physicians' orders are signed and implemented within 24 hours.

Review each client's MAR and/or EMAR regularly to review accuracy and to confirm that the appropriate signatures are included on their forms.

Document disposal of medication per procedure.

Assist in the development of policies and procedures/implementation of them.

Supervise Home Health Aides (H.H.A.'s)/Unlicensed Staff.

Supervise and discipline Home Health Aides (H.H.A.'s)/Unlicensed Staff.

Coordinate staff education to include at least 8 hours of in-services per year.

Provide health and medication training to all new employees and continued education for all personnel to assure compliance with state, federal and local regulations.

Assure that staff clearly understands and follows all Home Care procedures.

Monitor staff for compliance of monthly client treatments (as scheduled).

Supervise medication practices and medical equipment being used by home care clients.

Ensure that medication procedures are being followed correctly.

Facilitate coordination of client services with other service providers (as needed).

Participate/lead staff meetings, committees, etc. at the discretion of the Director.

Assist other departments with special events and family functions.

Be flexible and adaptable to changing situations.

Report injuries for self, staff, clients or visitors immediately to the Resident Director and/or the Director of Nursing.

Required Qualifications

Must be registered in the State of Minnesota as a Registered Nurse (R.N.) and must maintain a current and unrestricted licensure.

Must have a valid driver’s license. Driving may be necessary.

Ability to lift/carry objects weighing up to approximately 30 lbs. or more and to meet all physical requirements related to this position.

Preferred Qualifications

Experience in working with senior adults; knowledgeable and sensitive to the needs and concerns of the senior population.
