Collaborative practice including patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community.Process facilitates communication and care coordination along a continuum through effective transitional care management and utilization management.Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care.What You Need to Bring to the Table:
All Registered Nurses without a minimum of a Bachelor's degree in Nursing must obtain a Bachelor's degree in Nursing or higher within 3 years of hire.California Fire and Life Safety Card - National Organization within 30 days of hire.Able to work autonomously and be accountable for work with strong attention to detail.Demonstrate effective written and verbal communication skills, critical thinking and problem-solving skills, and the ability to resolve conflicts (including situations where there may be conflicting priorities).Excellent interpersonal skills regarding the hospital staff, patients, visitors, etc.Estimated Min Rate:
$35.00Estimated Max Rate:
