Program Support Assistant - Case Management

The case management associate participates on the Case Management Team (CMT), with the guidance of the nurse case manager and social worker, assists in facilitating appropriate patient placement; utilizes hospital and community resources for continuity of patient care; serves as Utilization Management liaison; collaborates with Medical staff, CMT members, Business Office, Medical Records and other hospital staff; performs other duties as assigned.Required Qualifications:1 year job-related experience in a healthcare settingPreferred Qualifications:Upon hire: Nursing Assistant Certification (CNA), Health Unit Coordinator (HUC), Medical TerminologyPrior experience in discharge planning, billing, utilization review, financial counseling or registrationProficiency in medical terminology
