OverviewHave you been curious about the organ transplant process? We at Fairview Health Services are seeking a Transplant Coordinator Heart Acquisition Transplant specialty to be based on the University of Minnesota Campus. This is a casual position meaning all hours are as needed.

Responsibilities Job DescriptionThe coordinator will coordinate care from referral for transplant, through the transplant evaluation to support transplant candidates meet their goal of listing on the waitlist and eventual transplant. Coordinators support patients in clinic, provide patient education on transplant, collaborate with referring providers and dialysis unit teams, review the inboxes in Epic and MyChart, respond to any urgent requests and deescalate emergency situations.The Transplant Coordinator is responsible for coordination of the long-term management of patients' health and wellbeing after transplant through collaboration with various members of the interdisciplinary team. They will work with team members within the organization and outside community partners while also acting as a liaison for transplant physicians to implement medical management of conditions. You may be asked to provide telephone triage and make patient care decisions based on patient symptom assessment and evaluation, laboratory results, and patient condition changes. We believe one of the most exciting responsibilities Transplant Coordinators have is providing education to the patients and their caregivers.We have found that those who excel in this role:Collaborate with other clinical team members to improve patient care and benchmarks through quality improvement projects.Actively chip in to the development and revision of protocols.Maintain knowledge of up-to-date practices in transplantation, nursing, and healthcare.

QualificationsRequired

BA or BS in Nursing3-5 years acute care experience in ICU, CCU, Transplant, Acute Care Med-SurgMN RN licenseBLSAdditional Requirements:

High standard of creative prioritization skills and priority setting. Shown focus on customer/patient service with every interaction. Excellent public relations and communication skills. Work as an effective team member to provide outstanding care. Ability to use appropriate computer applications. Ability to work independently, demonstrate critical thinking skills and engage in ongoing process improvement. Ability to accept change.
