Title: Resource Specialist

Orlando, FL and Maitland, FL
Note:
Individual will work from their home and will also be required to go into the field. Field visits could include home visits, visiting members in group home/hospital settings, or attending community/agency meetings.
Day to Day Responsibilities:This position will be field-based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and providing community resources to address social determinants of health needs. The caseload is 100 members. This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.
Required Skills/Experience:Pediatric case management experienceExperience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.
Preferred Skills/Experience:Health plan experienceFoster care and/or adoption experience
Education Requirement:Bachelor's Degree
Software Skills Required:Microsoft Office - Outlook, Word, and Excel
