During the 2020 pandemic, we also stepped forward as a regional leader in COVID care and testing.Job Description:The Behavioral Health Consultant supports Primary Care physicians/Health Home teams in identifying and behaviorally intervening with Health Home enrollees who could benefit from behavioral intervention.Minimum Requirements:Master's Degree in Social Work or demonstrated/verified equivalent in combination of education and experienceLicensed Social Worker - LMSW with a minimum of three (3) years experience working in a healthcare related settingGood written and verbal communication skillsMust be able to read and speak the English languagePreferred:Clinical Licensed Social Worker - LCSW
