Northern Light Eastern Maine Medical CenterDepartment: Urology ClinicPosition is located: Urologic Surgery of MaineWork Type: Full TimeHours Per Week:40 Hour/Week Work Schedule: Variable Summary:The Nurse Navigator serves as a consistent care coordinator for the diagnostic and surgical end of Urologic care to ensure timely access and seamless coordination of services throughout the continuum. The Nurse navigator will interface with the schedulers, internal and external providers, and assist in the diagnostic imaging, clinical evaluation, support services, and education of patients receiving services. The nurse navigator will be responsible for facilitating and coordinating the care of patients of USME that receive their care from providers working in collaboration with USME. He or She will identify and help eliminate barriers and refer to appropriate resources. Additionally, the nurse navigator will be responsible for the triage and coordination of referrals sent to our office. They will coordinate local support systems, arrange patient focus groups, and collaborate and provide educational resources for patients, families, and staff.
