Category Personal Care Description SUMMARYUnder the direction of the Manager of Case Management, the Intake Coordinator is responsible for overseeing the intake and assessment process for individuals with Intellectual and Developmental Disabilities (IDD). As the initial point of contact for individuals and their support team, the Intake Coordinator offers guidance and support throughout the intake process and serves as a key resource for those seeking information about available services. This role involves gathering and reviewing information, coordinating with healthcare and service providers, and ensuring a smooth transition into care or employment.Embrace Opportunity Village core values by cultivating positive and meaningful CONNECTIONS with individuals served and colleagues, demonstrating ENTHUSIASM, high standards of PRODUCTIVITY, authentic STEWARDSHIP, and consistent participation in individual and team DEVELOPMENT.ESSENTIAL DUTIES AND RESPONSIBILITIES Utilize appropriate methods for working with individuals with intellectual and or physical disabilities. Reinforce and positively engage in Opportunity Village's organizational culture Support choice and creativity of all individuals served and team members. Network and maintain professional, positive relationships with community referral sources. Participate in all case management activities, including: Conducting and participating in calls with community partners, i.e., Desert Regional Center, Job Developers (DRC), Clark County School District (CCSD), Clark County, etc. Conduct OV location tours Coordinate the intake assessment process, including: Scheduling meetings with all support team members Prepare assessment reports Communicate with service coordinators regarding all documentation needed, i.e., Social assessments and contract certifications Submitting all documentation and information to necessary internal departments, i.e., Human Resources, Service Contracts, Program Services, and external agencies, i.e., DRC and SLA Providers. Maintain current and accurate case files for each intake candidate. Ensure all communications adhere to HIPAA guidelines and privacy regulations. Answer and respond to telephone calls and e-mails. Maintain the filing system and computer records to ensure all information is secure yet easily accessible by the support team. Maintain all files to ensure compliance with Federal, State, Local, and Opportunity Village policies and procedures. Facilitate the weekly transitions team meeting. Maintain the agency waitlist for employment, programs, and services. Collaborate with Case Managers in the development of habilitation plans, support guidelines, and person-centered plans when applicable. Complete and submit scholarship applications and private pay agreements. During the intake process, serve as a primary liaison and advocate for individuals served. Participate in and attend special events (i.e. Community awareness events, internal and external hiring events). Report suspected Abuse, Neglect, Exploitation, Isolation, and Abandonment (ANEIA) immediately as mandated by state law May be assigned other duties as neededSUPERVISORY RESPONSIBILITIESThis position does not directly supervise others.Position Requirements SKILLS REQUIRED Ability to communicate effectively with individuals, team members, and supervisors (verbal and written). Must possess social perceptiveness to assess and understand other's reactions and behaviors. Ability to work independently. Ability to read, analyze, and interpret written information. Ability to define problems and draw valid conclusions. Ability to interpret instructions, gain knowledge, and have initiative. Ability to maintain strict confidentially. Ability to work with individuals with developmental disabilities. Must display a professional demeanor. Good organizational skills, ability to set priorities, and multi-task. Ability to work as a team player Possess strong customer service skills. Ability to resolve multiple complex issues simultaneously while remaining calm in stressful situations. Ability to conduct tasks and successfully perform under critical deadlines.EDUCATION AND EXPERIENCE Required: Associate's degree in Social Work, Psychology, or a related field, with one (1) year work experience supporting individuals with intellectual or developmental disabilities.Or High School Diploma (or equivalent) with three years' work experience supporting people with intellectual or developmental disabilities. Required: Proficient in Microsoft Office Suite, Excel, and Word. Required: Current CPI certification within 30 days of employment Required: Current CPR and First Aid Certification within 30 days of employment Preferred: Bachelor's degree in Social Work, Psychology, or a related field with Qualified Intellectual Disability Professional (QIDP) certification.PHYSICAL ABILITIES & WORK ENVIRONMENTThe physical demands described below represent those that a team member must meet to perform essential functions of the job successfully.
