Description
Position Summary:
The Prevention Specialist acts as the primary client advocate, keeps abreast of progress, and coordinates all services delivered to the client and family internally and through external referral.
Essential Job Functions:
Conduct Needs Assessment
Develop Service Plans in collaboration with the client and family
Conduct monthly Service Plan reviews
Document all clinical services and activities in accord with licensing, Medicaid, CARF standards and agency policy and procedure
Conduct individual case management sessions
Facilitates access to health insurance and public entitlements.
Perform activities on behalf of the client and family, including attendance at external appointments as needed
Provide referral and linkage for the client and family, as needed
Prepare monthly Progress Reports, as required, for assigned clients for courts, probation officers, caseworkers, etc.
Conducts HIV/VH test using OraSure or OraQuick and conduct pre/post counseling using CLEAR
Provides education about safer sex practices and substance use prevention
Coordinate client and family care with the treatment team and with external providers
Arrange transportation for clients, as necessary
Participate in case conference
Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation
Participate in meetings and attend trainings as required and requested
Completes all orientation and mandated training activities, including E-Learning Courses, within required timeframes, and in accordance with the criteria established for this position.
Perform any other duties as assigned.
Qualifications
Essential Qualifications:
Education:
Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field is required
Experience and Competencies:
Two years of related work experience in Mental Health, and / or Substance Abuse field preferred
Must receive CPS certification within 1 year of employment
Ability to meet requirements of Monroe County Sheriffs Office background check
CPR Certification, First Aid Certification and an annual Tuberculosis test.
Ability to provide an Affidavit of Good Moral Character
Clearance in Pre-Employment Drug Testing and Criminal Records Checks, as required.
During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.
