DescriptionResponsibilities:Follow a designated care plan in accordance with patient's needsMake home visits to clients in designated geographic territoriesPerform assigned duties, including administration of medication, wound care, treatments, and proceduresMonitor clients' conditions, reporting changes to Clinical or Client Services ManagerFollow up with, execute, and properly document doctors' ordersPerform client assessments as necessaryCase management and coordinationAccurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.Demonstrate caring and empathyTake and track vital signsMeasure and administer medications as prescribedProvide wound care, as neededProvide catheter careEnsure patient is fed and bathedEnsure bedding is cleanTrack and order suppliesWillingness to work a variety of shiftsReliable transportation to the residential settingMust feel confident providing nursing care without oversightAbility to create rapport with patient and family membersExcellent communication skillsEducation:Associate's degree in nursing, requiredBachelor's degree in nursing, preferred1+ years of nursing experience in a residential settingEducation: Bachelors Degree
