The Associate Vice President (AVP) of Post Acute Care Business Operations plays a pivotal role in the strategic alignment and operational excellence of Home Health, Hospice, and Durable Medical Equipment (DME) services. This leadership position is responsible for integrating network strategies across inpatient, outpatient, and post-acute care services, ensuring seamless care management and effective transitional care as patients move from hospital to home. In the realm of business operations, the AVP ensures that all facets of Home Health, Hospice, and DME services are optimized for performance and growth. This includes overseeing financial performance, enhancing service delivery models, and ensuring compliance with industry regulations. The AVP's strategic vision and operational acumen are vital to the organization's success, driving initiatives that not only enhance patient care but also position the organization as a leader in post-acute care services.

With a consummate focus on effective collaboration, business development, and leadership, the AVP of Post Acute Care Business Operations ensures that patients receive the highest quality care while enabling the organization to achieve outstanding operational and financial results.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, EXPERIENCE, AND/OR LICENSURE:

1. Bachelor's degree in Nursing (BSN), Healthcare Administration, Business Administration, or a related field.

EXPERIENCE:

1. 7-10 years of progressive leadership experience in healthcare.

PREFERRED QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Master's degree in Healthcare Administration (MHA), Business Administration (MBA), or a related field.

2. Advanced certification in healthcare management or quality improvement, such as Certified Professional in Healthcare Quality (CPHQ) or Fellow of the America College of Healthcare Executives (FACHE).

EXPERIENCE:

1. 10+ years of leadership experience in post-acute care settings, with a track record of successful program development and business growth.

1. Lead the strategic integration of Home Health, Hospice, and DME services within the broader network of inpatient, outpatient, and post-acute care services to ensure a seamless continuum of care. 2. Oversee the development and implementation of transitional care management programs, ensuring smooth patient transitions from hospital to home, with appropriate referrals and alignment to post-acute services.

3. Manage the day-to-day business operations of Home Health, Hospice, and DME services, ensuring efficiency, financial performance, and compliance with industry regulations.

4. Assess current operational processes and implement best practices to enhance service delivery, improve patient outcomes, and optimize resource utilization.

5. Facilitate collaboration across administration, medical, and nursing staff, fostering a culture of teamwork and shared goals to drive organizational success.

6. Utilize data analysis to inform decision-making, track key performance indicators, and identify opportunities for improvement in post-acute care services.

7. Oversee budget planning, financial forecasting, and cost management for Home Health, Hospice, and DME services, ensuring alignment with organizational goals and sustainability.

8. Lead the development and implementation of new programs and initiatives that enhance patient care, expand service offerings, and support business growth.

9. Ensure all services are in compliance with local, state, and federal regulations, as well as industry accreditation standards, including The Joint Commission (TJC) and CMS requirements.

10. Drive continuous quality improvement initiatives across post-acute care services, focusing on patient safety, care quality, and service excellence.

11. Provide leadership and mentorship to departmental managers and staff, fostering professional growth and ensuring alignment with organizational values and objectives.

12. Engage with key stakeholders, including patients, families, healthcare providers, and community partners, to gather feedback, improve services, and strengthen partnerships.

13. Optimize the allocation of resources, including personnel, technology, and equipment, to maximize operational efficiency and support strategic goals.

14. Monitor and enhance patient satisfaction by ensuring that care delivery is patient-centered, responsive, and meets the highest standards of excellence.

15. Identify and pursue opportunities for business growth, including expanding service lines, entering new markets, and enhancing the organization's competitive position in post-acute care services.
