QA Quality Assurance Specialist

to join our team. As a QA Quality Assurance Specialist, you will play a crucial role in ensuring the quality and reliability of our home healthcare services. You will be responsible for designing and implementing tests, identifying defects, and collaborating with teams to resolve issues to deliver seamless experiences to our clients.Competitive salary commensurate with experience.Opportunities for professional development and career advancement.Health insuranceVisionPaid Time OffResponsibilities:Provide training to clinical staff on CMS and State specific guidance regarding outcome measures, compliance with regulatory requirements and education.Establish and implement home healthcare focused quality and performance improvement efforts in accordance with systems used EVV, EMR, etc.Train and supervise team members to ensure documentation of timely, fiscally sound, medically necessary care our clients and organization.Partner with administrator and DON to support continuous accreditation readiness e.g. Mock surveys, Inter-Cycle Monitoring, Audits, etc.Monitor and assist in preparing individualized clinical reports to help clinical management team improve patient care and outcomes.Support Peer Review efforts for Nurses and Caregivers on quality data, accreditation and regulatory standards.Supports activities/tasks resulting from client safety incidents/findings.Review past incidences, claims and liability reports to identify the risks the office is facing.Gather national and statewide data to provide comparison and guidance to determine where company is and develop new policies and procedures for improvementProvide recommendations and solutions to immediate quality assurance problemsCreate a risk management plan and conduct risk management training to clinical staff to help avert future problems.Develop new policies and procedures.Requirements:Must possess a current, unencumbered, active license to practice as an RN in this state.Current CPR and first aid certification1 Year of Quality Assurance experienceExperience in OASISStrong knowledge of clinical operations, CMS and state regulationsManagement experience in the healthcare industryExperience interacting with multiple disciplinary teamsProficient in Microsoft Office SuiteExcellent communication, relationship building and interpersonal skillsExcellent Time management, organizational, and priority setting skillsExcellent analytical and problem-solving skillsStrong communication and collaboration skillsPreferred Qualifications:Certified Professional in Healthcare Quality Certification CPHQCertified Home Health Aide

The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The home health aide is responsible for observing patients, reporting these observations and documenting observations and care performed.The home health aide will be assigned by a Registered Nurse in a manner that promotes quality, continuity and safety of a patient’s care.

Director of Clinical Services

The Director of Clinical Services is responsible for the overall direction of the home health clinical services.

Licensed Practical/Vocational Nurse

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Registered Nurse (RN)

The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Therapist Careers

Certified Occupational Therapy Assistant

The certified occupational therapy assistant contracted through the Organization is responsible to the registered occupational therapist that is responsible for the implementation of standards of care for occupational therapy services and for adherence to all conditions outlined in the Service Agreement. The certified occupational therapy assistant performs only those services planned, delegated, and supervised by the registered occupational therapist.

Medical Social Worker

The medical social worker is responsible for the implementation of standards of care for medical social work services and as defined by the state Medical Social Work Practice Act.

Occupational Therapist

The occupational therapist contracted through the Organization is responsible for the implementation of standards of care for occupational therapy services and for adherence to all conditions outlined in the Service Agreement. Any direct patient care provided will be performed as defined by the state Occupational Therapy Practice Act.

Physical Therapy Assistant

The physical therapy assistant provides physical therapy services under the direction of the physical therapist and according to the physician’s plan of care.

Physical Therapist

The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

Speech-Language Pathologist

The speech-language pathologist is responsible for implementation of standards of care for speech-language pathology services and for adherence to all conditions in the Service Agreement.

Clinical Records Coordinator

Responsible for managing all aspects of maintaining and protecting organization clinical records including the management of the clinical records team.

Community Liaison

Coordinates and performs duties of marketing and communicating the role of the agency to the community.

Administrator

Responsible for planning, coordinating, managing and directing all activities and programs of the Human Touch Home Health Care Agency.

Referral/Intake Coordinator

Responsible for managing all aspects of the client intake process.
