What you'll do in this role:
Serves as the first line of customer service for external and internal customers.Professionally responds through electronic referral platforms within the specified time requirements.Performs reverse marketing in coordination with the local BD Leader to reinforce messaging, stay top of mind and present information where it may be of value.Meets assigned key performance indicators (KPIs).Receives and documents patient referrals to hospice services through electronic referral systems, ensuring compliance with applicable coverage criteria.Coordinates and communicates with Hospice Centralized Intake Coordinators or designee to enter referrals into Homecare Homebase.Gathers appropriate information to assist the clinical team in scheduling an evaluation visit.Provides the appropriate information from the eReferral system to the Hospice Centralized Intake Coordinator to ensure complete referral information.We are looking for compassionate Hospice Intake Analyst with:
High School diploma or GED equivalent.Experience working in an environment where timeliness and quality of response were measured and impacted business results.Hospice or Home Health experience, preferredExperience customer facing in sales or operations, preferredReliable internet service and ability to office from home with minimal disruption to the workday.Questions?
