We are currently seeking an Adult Prescriber who will offer psychiatric assessment and medication management within the outpatient Adult Psychiatry setting on a full time basis. This position is responsible for individual services to patients assigned by Crossroads Health. This licensed provider will engage as a member of a collaborative team. With a Trauma Informed approach, this position will be a representative for the organization and how we offer integrated services for recovery and mental health.
Additionally, the Prescriber will:Provide psychiatric assessments, medication evaluation and medication management.Regularly assess consumers & prescribe medications for clients via office and home visitsEducate consumers & their families regarding medications/symptoms/illness/side effects. .Participate in ongoing assessment and therapeutic engagement. Complete, as directed, elements of the comprehensive assessment for clients within 60 days of admission.Position Requirements:
Master's Degree (minimum)Licensed in the State of Ohio as an NP/ PACControlled Substance Registration CertificateMedicare Provider NumberProfessional Liability Insurance.Standard Care Arrangement (annually) or Supervision Agreement (whichever applicable)BLS/First Aid (employer provided)Must have valid driver's license
