Our newly renovated, dedicated Respiratory Care Unit provides a person-centered approach to caring for our residents. All staff receive specialized training approved by the state board. Sapphire Health Services is currently seeking a dedicated Registered Nurse to join our team and uphold our commitment to excellence. JOB SUMMARY:A Registered Nurse (RN) working in the Vent unit of our Skilled Nursing Facility is responsible for delivering high-quality, person-centered care to residents, coordinating with interdisciplinary teams, and ensuring compliance with all regulatory standards. This role involves assessing resident needs, implementing individualized care plans, administering medications and treatments, and providing leadership to nursing staff. The RN plays a crucial role in enhancing the health and quality of life for residents. Ventilator, trach care and vent settings will be managed by RT staff 24 hours a day along with RN PRN support. RESPONSIBILITIES:Conduct comprehensive assessments of residents upon admission, change in condition, and periodically as requiredProvide primary care to residents in our Vent UnitSkills such as: wound care, enteral feeding, line care, diabetic managementParticipate in care plan meetings with residents, families, and the care team to discuss goals, progress, and necessary adjustmentsProvide direct nursing care to residents, including administering medications, treatments, wound care, and other necessary interventionsMonitor and document residents' health status, including vital signs, symptoms, and responses to treatmentAssist with activities of daily living (ADLs) as needed, such as bathing, dressing, grooming, and feedingEnsure all care provided is consistent with the resident's care plan and nursing standards of practicePrepare, administer, and document prescribed medications and treatments accurately and safelyMonitor residents for adverse reactions and medication effectiveness, and report concernsMaintain accurate and up-to-date medication administration records (MARs) and ensure proper storage and handling of medicationsServe as a liaison between residents, families, physicians, and other healthcare professionals to ensure effective communication and continuity of careReport significant changes in resident conditions promptly and take appropriate actionParticipate in shift change reports, staff meetings, and interdisciplinary team meetings to ensure cohesive care deliverySupervise and provide guidance to Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), Certified Medication Aids (CMAs) and other support staffAssist in the orientation and training of new nursing staff to ensure compliance with facility policies and proceduresDelegate tasks appropriately and oversee the performance of assigned duties by nursing support staffMaintain accurate, timely, and thorough documentation in residents' electronic health records (EHR) or paper charts as requiredEnsure compliance with federal, state, and local regulations, as well as facility policies and proceduresAssist in preparing the facility for regulatory inspections and audits by maintaining high standards of care and documentationParticipate in Quality Assurance and Performance Improvement (QAPI) programs to identify areas for improvement in resident care and safetyFollow infection control protocols, including proper hand hygiene, use of personal protective equipment (PPE), and disposal of biohazardous materialsReport and document any incidents, accidents, or safety concerns promptly and accuratelyAdvocate for the rights, preferences, and needs of residents, promoting a respectful and compassionate care environmentOther Duties as AssignedREQUIREMENTS: Registered Nurse (RN) Licensure in the State of practice Previous experience in a skilled nursing facility, long-term care, or acute care setting preferredStrong clinical skills and knowledge of geriatric careExcellent communication, interpersonal, and organizational skillsAbility to work effectively as part of an interdisciplinary teamProficiency in electronic health records (EHR) and basic computer skillsCritical thinking and problem-solving abilitiesAbility to perform physically demanding tasks, such as lifting, bending, standing, and assisting residents with mobilityMust be able to stand and walk for extended periodsAt least 1 years of Vent/Trach experience preferredBENEFITS:Benefits of being a Sapphire Health Services employee include, but is not limited to:401K Medical, Dental, VisionBirthday/Holiday PayWellness FundUniform FundLongevity BonusesQuarterly Vacation DrawingTuition ReimbursementHOW TO APPLY:At Sapphire Health Services we are dedicated to creating a supportive and enriching environment for both our residents and our team members.
