Home Care Case Manager | Aide Supervisor NEEDED For Home Care Agency In MN

We are a home care agency that has been in business since 2007 and rapidly growing in 15 states. We provide assistance with activities of daily living for the elderly (i.e., help with bathing, dressing, grooming, toileting, etc.). We are highly respected across the 15 states we service and looking for quality people to join our team.

We are currently in need of a Home Care Agency Case Manager for our MN operation. This position will be available to work remotely after 1 month of training in our Bloomington, MN, or Fargo, ND Office.

MUST

have experience working with seniors in Home Care or Assisted Living.

Bachelor's Degree preferred.

Healthcare benefits available after 60 days. Starting salary is $95,000. Profit Sharing Bonuses available for high performers. A 1% increase in salary for each milestone increase in revenue the company makes is available to employees who are managing their duties with high performance. 10 such 1% increases have been average per year. In addition, this position is eligible to receive $1.25 per hour bonus for each hour of care each family member they hire to be a client's caregiver. Our contract with the Department of Veterans Affairs allows us to hire family members/friends to be the Veteran's caregiver and this position is responsible for onboarding such and getting them through some of the steps to hire/training. This position will then receive $1.25 per hour for each ongoing hour of work that family member/friend caregiver works in perpetuity with our organization. There is no cap on the amount of money that can be received via this bonus.

The job duties include supervising home care aides and coordinating care with clients. Phone calls, email, and Zooms are how all communication occurs for this position. No on-site visits required. This position requires average level Microsoft Office skills - as the position uses Microsoft Outlook and Microsoft Word on a daily basis. The usual work schedule will be from 8:30 am to 5 pm Monday - Friday - but this doesn't mean that there will be days when work spills over later into the evening or weekend - as the job is not done until the job is done each day. We are looking for people who have capability in their schedule to work late from time to time when needed.

We are not a bank open Monday - Friday from 9 am to 5 pm. We are a healthcare company that provides care 24x7 to our valued clients. This means things can sometimes happen after regular business hours. We have systems and processes in place to handle such situations - but we do expect our Case Managers to be able to work late if needed to ensure their caregiver knows how to use the app on their phone so they can pull up the client's address the next morning when they are going to work - so the client doesn't go without care because the caregiver couldn't log into their app to get the client address, etc.

Little things like this are important for the smooth operation of our company and to provide the Gold Standard of Care that our customers and referral sources have come to rely on. Again, the job is done when the job is done. We are not saying that this job is an on-call, all day/night, 7 days a week job - but we are saying that it is not a come and do your 8 hours and leave and never think about your job until the next morning. There may be times you need to come back to your home office later in the evening to finish up work to ensure a client does not go without care the following AM. That is the reason we allow working remote - so it makes doing such things easier on the Case Manager... instead of having to drive to our office to do such.

The position is responsible for doing an on-call shift every 20th weekend. The on-call shift can be performed remotely at home and requires being available to answer home care aide questions that come in via text and phone - during the hours of 9 am to 5 pm Saturday and Sunday - to ensure client care needs are met over the weekends.
