9.28.2023SummaryUnder the direction of the Inside Safe Supervisor/ Program Manager, the Motel Resident Case Manager serves as a point of contact for the individuals and families currently enrolled in SSG/HOPICS Inside Safe Motel Voucher Program. The incumbent will provide support at the motels, provide conflict resolution to any issues that may arise while clients are at the motels, and provide referrals to additional services as needed.Essential FunctionsCoordinate with other project staff and partners to provide housing placement services and supportive services to facilitate clients' successful transition to permanent housing and coordinate services to foster housing stability during and beyond Time Limited subsidy.Staff must be available to work weekends and evening shifts.Conduct wellness checks for inside safe participants at various motels to ensure safety, provide case updates, and enforce program and motel rules.Complete incident reports as needed and submit them to the program supervisor within 24 hours.Provide weekly check-ins for clients to assist them in locating permanent housing.Create the participant’s record in HMIS at the same time the participant is determined to meet program eligibility requirements.Update the participant’s housing status within twenty-four hours of any change.Track services and referrals, and complete case notes on services provided within 24 hours following the provision of the services.Create a Status Assessment Update within 24 hours of any significant changes during the client's enrollment in the program.Enter exits for participants enrolled in any Interim Housing programs into HMIS the same day.Update exit destinations for participants who have exited to permanent housing.Enter case notes within 24 hours in DAP form, ensuring they are detailed and grammatically correct.Address data errors reported weekly and make necessary corrections within the provided deadline.Submit Daily Deliverables Report.Submit weekly Productivity Reports to the direct supervisor on Fridays.Provide conflict resolution at motel sites.Address motel manager concerns regarding participants.Attend weekly case conference meetings.Collaborate and coordinate with DMH, DPSS, LAUSD, WorkSource, and SUD providers.Maintain up-to-date and accurate documentation in client files for service coordination/case management.Provide the Program Manager with all required client information and assessment outcomes.Enter data into HMIS within 24 hours.Prepare project reports in accordance with funding requirements.Assist in training and providing support to necessary staff and project partners.Conduct criminal background checks on all pre-eligible applicants and household members over 13 years of age.Maintain files/records on client services in compliance with HIPAA and other funding requirements.Maintain appropriate boundaries and adhere to SSG’s Code of Ethics and HOPICS’ Core Values.Represent the Agency in a professional manner at meetings and community events.Build trusting relationships by acting with integrity, courtesy, and responsibility.Home visits required.Regular attendance required.Other duties as needed.Secondary FunctionsPerform other duties as assigned by the Supervisor, Program Manager, and/or Division Director. Answer phones and route incoming calls; serve as back-up to the Case Managers in their absence.Minimum Qualifications - Knowledge, Skills, and Abilities RequiredAn associate degree in social services or other related fields is required, with a minimum of one year of job-related experience working with homeless individuals and families, OR five years of experience in the social service field; case management and homeless program experience preferred. If in recovery, a minimum of three years of being drug and alcohol-free is required. Working knowledge of Microsoft Word, Excel, and other database programs is necessary. Ability to communicate effectively, both written and orally, and work with clients from diverse backgrounds is essential.Non-Essential Qualifications:Experience using HMIS and at least one year of rapid re-housing experience preferred.Supervisory Responsibilities:This position does not have any supervisory responsibilities.Environmental Conditions (Working Conditions)This position is responsible for working in “client-friendly” environments and is required to visit other shelters and homeless access centers as part of their duties. Local automobile travel is required.Physical RequirementsThe Case Manager typically spends time sitting, standing, typing, thinking, writing, walking, driving, and carrying (max. 25 lbs).Mental RequirementsThis position will require the individual to handle constant distractions and interruptions, and to process information effectively.Special Service for Groups is an Equal Opportunity/Affirmative Action Employer
