Position Details:
Hartford HealthCare is seeking a per diem Internist, Family Medicine or Geriatrician on a per diem basis to provide Skilled Nursing Facility (SNF) admissions and rounding coverage.
Role: Geriatrician
Location(s): SNFs in the Bloomfield area
Setting: 100% Skilled Nursing Facility (SNF) –patient admissions and rounding
Schedule: Per Diem, as needed (seeking 4-5 hours/day, 3-4 days a week)
Call: No call

Supported by team of APP’s that are in the facilities full time within a team of 18 Geriatric providers Strong support staff, nursing and operational team plus strong peer support to assist with any challenging situations that may arise Our system offers a robust compliment of senior resources and a Geriatric Council that our team works closely with to provide a personalized, coordinated care Hartford HealthCare is one of the largest, non-profit, fully-integrated healthcare systems in Connecticut, allowing our providers to care for their patients no matter where they reside in Connecticut
