Job Responsibilities

Delivering post-acute care as an LPNMonitoring the physical and mental wellbeing of residents and tracking their vital signsPerforming intakes of residents and documenting their health concerns and medical historiesReviewing the medical records and charts of residentsProperly administering (and verifying) medicationsHelping to maintain appropriate levels of medical suppliesContributing to a safe, comfortable environment for residentsAssisting with IV set up, vaccinations, and phlebotomiesBeing accountable to follow and adhere to care plans for residentsHelping medical staff with orientation and training

Qualifications and Skills

Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN)Completion of approved Licensed Practical Nurse (LPN) educational programHigh school diploma (or GED/equivalent)Minimum of 6 months of experience (LPN or related role)Ability to lift a minimum of 50 lbs
