Growing in-home assessment group seeks a Nurse Practitioner / NP to work PRN in the Indianapolis, IN area New grads welcome Join a team that delivers exceptional personalized care wherever patients call home About the Company: Founded in the early 2000s, this group cares for Medicare Advantage, Managed Medicaid, and Commercial beneficiaries in the home acting as the bridge to care by connecting patients to the resources and support they need The in-home, comprehensive health assessments include diagnostic testing, risk identification, medical management, and tailored lifestyle improvement plans. Benefits: Compensation $90 per completed assessment 401K and limited voluntary benefits Mileage reimbursement plan About the role: PRN - flexible, create you own schedule Create your own schedule, 20 assessments/month, 45-60 minute assessments Contact Center will build provider's schedule based on their availability APP will travel up to 120 miles per work day, mileage will be reimbursed Physician Assistant / Nurse Practitioner will work autonomously to see Medicare age patients in the home, performing annual comprehensive health assessments Background: Board Certified Family Nurse Practitioner / FNP, Acute Care Nurse Practitioner / ACNP, or Adult-Geriatric Nurse Practitioner / AGNP Ideal background is a Nurse Practitioner with urgent care or ambulatory experience Open to new grad Nurse Practitioners FOR MORE INFORMATION: Katie Patterson 843.574.8234 katienowhealthcare.org
