Advanced Practice Provider
to join our Neurovascular team in Urbana, Illinois.Carle Health is a Comprehensive Stroke Center providing a full spectrum of neurovascular care. We have over 800 stroke admissions per year. The Neurovascular program at Carle consists of four physicians who are actively involved in neurointerventional procedures for aneurysms, stroke, AVMs, carotid disease, etc. We welcome candidates to join our busy and rewarding program to care for complex neurovascular patients. We also provide interventional support to patients with spinal fractures requiring kyphoplasty/vertebroplasty.Position Details Include:Responsible for helping to care for patients diagnosed with brain aneurysms, subarachnoid hemorrhage, acute stroke, and other neurovascular diseases, in collaboration with physician colleagues.Perform pre-procedure risk stratification including History and Physical Examinations for patients undergoing neurovascular procedures, consultations, and provide post-procedure follow-up visits in the inpatient and outpatient setting.Manage the patients post-procedure in the inpatient setting as well as co-management of patients in the ICU; they will perform follow-up visits and new patient evaluations in the outpatient clinic.Evaluate patients undergoing kyphoplasty/vertebroplasty, and provide support to outpatient follow-up as needed.Provide continuity of patient care by facilitating and coordinating communication between the health care team, the patient, and the patient's families.Develop and implement patient care plans, including discharge planning and coordination of care with other healthcare providers.Typical schedule M-F; with rotating weekend coverage every 5 th
weekend. Weekend coverage will include management of
neurosurgical
patients (cranial and spine) with supervision by staff neurosurgeon.Education Requirements:Master's degree in Nursing or Physician Assistant.Licensure/Certification Requirements:Current Illinois registered nurse license and advanced practice nurse/physician assistant licensure.
