One Medical Seniors wants to restore humanity to healthcare, for both patients and team members.The OpportunityThe Team Nurse (RN) is a key member of the clinical team in each Senior Health clinic and partners with the other members of the clinical team (physicians, health coaches, Behavioral Health Specialists) to provide accessible, comprehensive, coordinated care based on longitudinal healing relationships. As the Registered Nurse, youll report to the Office Manager. The Nurses primary responsibilities center around the delivery of clinical nursing care, patient education, and achieving continuous excellence in a primary care setting. Our clinics provide broad, comprehensive primary care and the Nurse has a key role in ensuring we are successful in this endeavor.What you'll likely work on:Provide direct patient assessment and care for adults 65 and olderPhone and walk-in triage for patientsOversight of anti-coagulation clinic activities and patient monitoringTransitions of care coordinator (post-discharge visits, discharge calls, etc.)Participate in the clinical development of Health CoachesParticipate in process and quality improvement initiativesPerform IV initiation, drug therapy management, and phlebotomy (as needed)Administer IM/SQ/and PO medications and vaccinesProvide treatments and assist providers with proceduresPerform diagnostic tests (including but not limited to: EKG, ABI, Spirometry)Oversight of in-house laboratory processingPerform point of care testing (Hemoglobin A1c, INR, etc)Provide patient teaching, education and supportParticipate as an active care team member across multiple disciplinesWound careAble to travel to surrounding clinics in

Total compensation packages may be based on factors unique to particular candidates, such skill sets, depth of experience, and work location.
