**Job Description:**

- Responsible for care coordination and care transitions planning throughout the acute care patient experience.- Collaborate with physicians, utilization managers, medical social workers, and bedside RNs to ensure timely progression and transition of patients to appropriate levels of care.- Facilitate communication and care across the continuum through effective resource coordination to prevent unnecessary admissions or readmissions.- Achieve optimal health, access to care, and appropriate utilization of resources balanced with patients' self-determination.- Conduct initial and ongoing reviews of physician admission care plans, gathering medical, psychosocial, and financial information.- Screen for ancillary supportive services, including Palliative Care Services needs.- Lead the healthcare team in developing comprehensive, cost-effective care coordination plans.- Identify and resolve barriers to care progression and transitions in a timely manner.- Collaborate with financial counselors for delivery of inpatient stay denials.- Ensure compliance with state and federal regulatory requirements for transitions and post-acute transfers.- Participate in patient rounds and collaborate with the interdisciplinary team for timely transitions.- Maintain current knowledge of CMS, Joint Commission transitions of care requirements, and other regulatory standards.- Communicate effectively with patients, families, and the healthcare team while addressing complex clinical and social situations.- Make appropriate care plans and referrals to community resources for high-utilization patients.- Participate actively and positively in department operations, fostering a collaborative environment.
