ConcertoCare Assistant Medical Director

manages clinical care delivery and performance for a team of Advanced Practice Providers in collaboration with the Medical Director. Key activities include direct patient care, support of clinical care delivered by field-based clinical practitioners, data-driven and KPI-directed clinical performance management, relationship support for key partnerships, and daily personnel management for clinical practitioners. Be a part of our pioneering, value-based clinical model that is redefining patient care for those who need it most.What you will do:Engage in meaningful, top-of-license clinical practice, ensuring the best outcomes for our patients. Direct patient care including annual wellness, continuity care, urgent care, and care transitions visit types in home, clinic, congregate living, and/or skilled nursing facility-based settings.Guide and mentor a dedicated team of Advanced Practice Providers, ensuring optimal care delivery and performance.Management of local provider teams including hiring, training, coaching, counseling, supporting, and evaluating direct reports.Onboarding support and clinical training for new clinical team members.Participation in care team huddles to provide clinical input and support individualized care plan development.Clinical performance management including ongoing clinical data evaluation and strategic deployment of clinical team resources to achieve KPI targets.Collaborative partnership with market leaders including engagement in strategic design, planning, and implementation of data-driven initiatives, processes, and programs to meet performance targets.Clinical liaison for local clinical community partnerships to support individual patient care needs and to ensure successful ongoing clinical collaboration.Engagement in the development and implementation of new clinical initiatives.Adherence to medical policies, protocols, criteria, and clinical guidelines to ensure best practices are maintained.Participation in the after-hours on-call schedule as required.Other organizational and/or market-specific duties as assigned.Requirements:MD or DO with graduation from an accredited health professions school and active national board certification.Active licensure in California.Active DEA licensure and CA state-specific controlled substance registration, as applicable.Certification in basic life support (BLS) is required.Clinical experience caring for aging adults and adults with complex care needs is required.Average to advanced computer and software skills required.Ability and means to travel as needed; proof of liability and property damage insurance on vehicle used is required.Special Consideration for:3+ years of clinical practice experience as a provider preferred.Advanced training in geriatrics and/or palliative medicine preferred.Clinical experience caring for patients in the home setting preferred.Clinical team leadership experience preferred.Previous experience with managed care and/or value-based healthcare delivery preferred.Base Salary/ Wage Range:

$250,000 to $275,000 plus annual bonus.
