Salary: $100,000 - $119,000 per year

We are Hiring a Hospice RN Referral Liaiso to work ONSITE at one of NYC's most prestigious Hospitals
Essential Job Functions
- Collaborates with physicians, medical staff, or clients in the Hospital to evaluate eligibility for Hospice services
- Communicates with discharge personnel in assigned hospital to ensure a smooth and satisfactory transfer of the patient to the home or skilled facility setting.
- Educates physicians, discharge planners, and social workers about Agency services and the general benefits of hospice services.
- Prospects for and identifies new physicians/discharge planners who have potential to refer patients to Agency for hospice needs
- Conducts initial and ongoing needs assessment of patients in hospital and responds accordingly, or refers on to the appropriate department that can address and/or meet those needs
- Attends and assists in enhancing effectiveness of liaison meetings; serves on agency committees.
- Assists in creating the production of printed educational material and brochures regarding services provided and presents material to patients
- Participates with other Health Care Liaison team members in co-marketing and coordination of Hospice services
- Creates and maintains records and files of all referral contacts
- Meets and/or exceeds referral and admission quota expectations for assigned location onsite at Partner Hospital
Requirements
- RN with active NY State Nursing license in good standing
- Minimum of two (2) years healthcare, home health, or hospice liaison experience preferred
- Knowledge of standards of practice for all services provided
