JOB DESCRIPTION

Our Palliative Care providers manage chronic debilitating illnesses and endof life issues. Provide cross coverage asneeded/requested by nursing and medical staff. Round on assigned medical hospitalist patients and on patients asrequested by facility medical staff.

Core responsibilities:

Conducts family meetingsPerforms consults and follow up visits as neededCreate, direct, coordinate, and supervise the patient care plansdailyConduct/order diagnostic tests, and analyze results and prescribeindicated treatments and therapiesRound on existing hospitalist patients in multidisciplinary roundsif askedWork as part of an integrated team with care management, nursing,pharmacy, etcAssist in patient education and goal settingParticipate and adhere to all quality metrics and goals as establishedand directed by Adfinitas or the client/hospitalSubmit accurate and timely charges for all professional servicesperformedAttend committee, CQI and management meetings, as appropriateCOMPETENCIES

1+ years of experience as anAdvanced Practice Provider in an acute role, preferably long-term care orskilled nursing facility or similar inpatient experience requiredGraduateof an accredited school and program, respective to role (PA-C, or NP)Licensureto practice medicine in good standing, with the state governing bodyFederalDEA certification in good standingStateCDS in good standingPossess strong clinical skills and demonstrate competency inproviding comprehensive health care including evaluation, diagnosis andtreatmentFlexibility and organizational skills to work autonomouslyProven skills, knowledgebase, and judgment necessary for independent clinical decision makingABOUTUS - ADFINITAS HEALTH
