About Us
BoldAge delivers innovative, creative solutions for PACE (Program of All-Inclusive Care for the Elderly). Our model of care enables older adults to live a meaningful, independent life at home with grace and dignity. We meet the evolving needs of older adults by providing innovative disciplinary solutions that consider both their physical and mental well-being. Our owner operated model is aimed at expanding the ability for older adults to live happily and healthfully in their own homes for as long as possible.
JOB SUMMARY
Under the direction of the Executive Director, the Director of Outreach and Engagement (OED) is responsible for census growth of the program by ensuring the PACE organization is integrated into and fully understands the community it serves, and participants are accurately enrolled in the PACE program.
The OED promotes and maintains high-quality services according to BoldAge values, PACE principles and best practices. The OED develops the PACE organization’s network of business, healthcare, and community relationships by:
Identifying strategic and key referral sources in the community.
Tracking referral activity to maximize marketing resources and resource allocation of the team actively participating in community groups.
Presenting at community events about PACE and care of older adults.
Maintaining relationships with both in-network and key non-network health care providers.
Supporting other PACE staff in networking in the community.
The OED is responsible for the successful intake and enrollment of PACE participants including developing and administering procedures that are highly effective at:
Responding to the needs of the community, especially older adults, and their families.
Establishing relationships with potential enrollees and their families.
Meeting state and federal requirements.
Facilitating the assessment and eligibility determination processes.
Ensuring an effective and satisfying transition for each participant and family member into the PACE program.
The OED develops effective strategies that promote continued growth including:
Person-centered and highly efficient processes.
Tracking, reporting, and analysis of all relevant data, trends, and statistics.
Internal staff involvement and communication.
Understanding the needs of the community.
Participating with the PACE management team in strategic planning to meet these needs.
REQUIREMENTS
A baccalaureate degree in health is required; Master’s Degree is preferred.
Professional license and/or certification is preferred (NHA, RN, LSW/MSW, PT, OT, RT, RD).
Experience in community outreach, intake, enrollment, or marketing preferred. Management experience in a human services organization preferred.
Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
