Abbott House, LLC is a Specialized Mental Health Rehabilitation Facility (SMHRF) located in the heart of downtown Highland Park. General Job Description:

1. Assure quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital and nursing division's philosophies and standards of care set by Illinois Board of Nursing, Illinois General Assembly Nurse Practice Act, and other governing agency regulations.

2. Communicate with interdisciplinary team any changes in condition of residents that may require change in plan of care.

3. Communicate with D.O.N. and/or physician changes in condition of person served to obtain appropriate orders for continued care.

4. Utilize the Communication Log, daily.

5. Participate in orientation of new nursing staff employees.

6. RNs may act as Qualified Mental Health Professional (QMHP) if the QMHP is absent from the facility; LPNs may act as Mental Health Professional (MHP) if the MHP is absent from the facility.

7. Demonstrate computer proficiency and efficiency with use of electronic records.

8. Complete monthly nursing summary, nursing goals for Care Plans, quarterly assessments for medications, and ongoing progress notes.

9. STAT labs including specimen collection, ordering x-rays, EKG, etc.

10. Operate within the nursing scope of practice in line with principles of best practice and clinical governance.

11. Complete Infection Control Report for any resident prescribed antibiotic.

12. Log the destruction of expired and discharged medication in the log book. Another nurse must witness the destruction of controlled substances and both nurses must sign the Destruction of Medication Log Book.

13. Complete POS and MAR for half of the residents on the floor on the 14th of each month.

14. Assume the lead role when responding to medical and psychiatric emergencies.

15. Follow protocol for hospitalization of a resident in an emergency.

16. Follow protocol for infection control, needle safety, etc.

17. Schedule appointments and transportation for resident appointments outside of the facility.

18. Log refrigerator temperature in log book.
