*Highland Care Center,* a 320 bed Skilled Nursing Facility is currently seeking dedicated *Licensed Practical Nurses (LPNs) *to join our staff. We are a skilled nursing and rehabilitation facility that is dedicated to helping our residents maximize their potential and live their lives to the fullest. *Shifts available:* Full-time evenings, nights and weekends.
*Base pay is $30.13-$41 per hour and evening and night shifts are eligible for a 10% shift differential. *

*Summary of the Licensed Practical Nurse (LPN):*
Provide nursing care to residents as prescribed by the physician and in accordance with standards of nursing practice and regulations and as directed by the Director of Nursing.
*Essential Duties & Responsibilities of Licensed Practical Nurse (LPN):** Assess and document resident progress in the medical record.* Obtain changes in physician’s orders as indicated by resident’s progress or lack of progress* Communicate pertinent information about resident condition and progress to charge nurses, supervisors and Director of Nursing, as indicated.* Administer medications and treatments accurately as ordered by the physician and according to all facility policies and procedures.* Assist in monitoring the inventory of medications, medical supplies and equipment to be sure adequate supplies of skin care products are on hand to meet the needs of residents.* Participate in educational programs for residents and their family members relative to skin care when a resident is being discharged to home.* Participate in Quality Improvement activities, as assigned.
*Job Requirements of Licensed Practical Nurse (LPN):** Licensed Practical Nurse current state license in New York.* At least one (1) year of Skilled Nursing experience preferred* Must have a current/active CPR certification.* Excellent technical, assessment, and documentation skills.* Effective verbal and written English communication skills.* Customer service oriented with the ability to work well under pressure.* Ability to work with minimal supervision, take initiative and make independent decisions.
