We are offering a $6,000 Sign-on Bonus (to be paid throughout the first year of employment).

Lutheran SeniorLife provides a comprehensive array of community based health and social service programs throughout western Pennsylvania.

Our services include LIFE (Living Independence for the Elderly); skilled nursing, home health; personal care; adoption & foster care; assisted living; adult day services; memory care; hospice; palliative care; residential living; affordable housing; home safety monitoring; Meals on Wheels; rehabilitation services and private duty.

LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.

Under the supervision of the Director, Clinical & Home Care Services and working closely with the Interdisciplinary Team (IDT), the Home Care RN provides the technical, organizational, and communication skills necessary to efficiently assess the need for and coordinate delivery of home care and clinic services. The Home Care RN must demonstrate the knowledge and skills necessary to assess, plan care for and provide service to frail elders according to assigned responsibilities and LIFE-Program standards.

Assessment and Care PlanningTakes part in each assigned participant's assessment and reassessment, including initial and periodic, and the participant's plan of care.Is knowledgeable about participant's overall needs and views the participant in a holistic manner.Assesses participants' condition, functioning, and environmental safety in the home, as well as, the clinic, utilizing the nursing process.Personally assesses the participants in the home during his or her evaluation for the program.Assesses all participants according to policy.Participates in the development and revision of the participant's plan of care and 24 hour service delivery as part of the IDT.Provides necessary documentation for and attends intake/assessment meetings for assigned enrollees.Develops, revises, and implements effective assessment planning tools for home care.Coordination

Coordinates the implementation of home care services including nursing, personal care, rehabilitation, supplies, and durable medical equipment.Ensures completion of authorizations for necessary services authorized by the team.Orients in-house and contracted staff to the participants being served and his/her needs.Coordinates care planning with other nurses, medical staff, and hospital or nursing home case management to minimize or eliminate unnecessary inpatient care and accomplish smooth transition to and from home.Establishes and maintains effective working relationships with program staff, contractors, participants, families, and other service providers.Evaluates the effectiveness of home care provided by in-house staff and contractors.Participates in quality assessment and improvement activities as directed.Works with Director, Clinical & Home Care Services to prioritize home care to meet the needs of participants in a cost effective manner.Communication

Counsels and educates participants and caregivers/family members towards self-help and recognition and resolution of medical, emotional, and environmental health problems.Regularly informs the IDT of the medical, functional, and psychosocial condition of each participant.Remains alert to pertinent input from other team members, participants, and caregivers.Documents care provided in a timely manner, according to accepted professional standards and the organization's medical records policies.Documents changes in a participant's condition in the medical record according to policy and procedure.Nursing

Provides skilled nursing care and prescribed treatments, and demonstrates nursing care to participants and families in the participant's home or in the clinic.Evaluates and ensures appropriate administration of medications in the home and/or in the clinic.Demonstrates nursing care in compliance with accepted professional standards and the organization's policies.Rotates on-call duties with other nursing staff, to troubleshoot, advise, teach and coordinate participant care, including emergency services.Other duties as assigned.Training

Assists in the training, competency testing, and evaluation of other nurses and paraprofessional staff.Orients other staff and providers to the participants and teaches proper care in the home setting.Teaches or addresses staff, participants, or other groups related to nursing and health.Requires license and current registration to practice as a Registered Nurse in the State of Pennsylvania. BSN preferred.TRAINING: On an annual basis employee will have completed training including:

the need of the participant in the center's targeted population,body mechanics/transfer techniques,voluntary reporting requirements involving abuse, neglect and exploitation,positive approach methods to manage behavior,and two (2) sessions, (totaling a minimum of eight (8) hours), targeted at enhancing the quality of care given and the employees job performance.OPERATING PRINCIPLES.... Must possess a current, valid Registered Nurse (RN) license for the state of Pennsylvania. Possess a Registered Nurse (RN) degree from an accredited college, university, associate’s degree program, or diploma from an accredited school of nursing.Minimum of one (1) years' experience working with the frail or elderly.Working knowledge of computers, Microsoft Word and Excel required.
